Prescription Renewals
If you need a refill on your medications before your next appointment, please try to give us at least five days' advance notice. The following instructions will assist you in having the information you will need to renew your prescription.
Have your medicine bottle on hand so you can provide the necessary information:
Cancellations
Call the office as soon as possible if you are unable to keep an appointment. Failure to cancel an appointment prevents other patients from being seen and increases the cost of care. If you are more than 15 minutes late, your provider may have to ask you to reschedule your appointment.
After Hours Emergencies
Dr. Randall is available by emergency beeper 24 hours y days a week. Please call the office phone number to leave a message for referral information, prescription renewals, non-urgent medical questions, and billing questions. Dr. Randall can also respond to questions and lab results by email, which will be answered within 48 hours. If it is a true emergency and Dr. Randall needs to be reached, please follow the instructions on the message. House calls are available in medically necessary situations.
Dr. Randall admits patients to University Hospital at Stony Brook. All Emergency Room or Hospital visits must be reported to this office within 24 hours.
Transferring Medical Records
The practice acknowledges the right of patients to transfer their care to another doctor or practice. A fee is charged for this service and an account will be issued upon receipt of the request. Once the account has been settled the appropriate medical history will be forwarded. The original records remain the property of the practice, and a copy of the requesting letter is kept on file in the patient record.
The practice recognises the right of patients to seek a further opinion and will provide a referral where appropriate.
If a new patient to the clinic requests their medical records from their previous doctor, a pro-forma request form is available on the patient's computer file for the patient and new doctor to complete.
Patient interviews will be conducted quietly and discreetly to ensure confidentiality at all times. The waiting room contains background music and a television to help provide auditory privacy.
All staff are briefed upon commencement of employment of the consequence of breaches in confidentiality. The signed employment contract states that the employee will be instantly dismissed if confidentiality requirements are not adhered to.
